Criteria To Be Considered For Eligibility To Advance Beyond Level One and To Any Higher Level
REQUIRED: Clinical candidates letter of intent to manager.
REQUIRED: Must be in good standing with letter of support from manager.
REQUIRED: Current (and up to date) professional resume.
REQUIRED: Two letters of recommendation: 1 from MD/DO or NPPA and 1 from clinical peer.
REQUIRED: 2-page personal narrative outlining why the candidate should be considered for progression in the clinical transformation ladder.
REQUIRED for Competency Eligibility: Supporting documentation of one or more of the following elements (Refer to Competencies A, B, and C)
A. Quality Improvement/Lean Improvement work
B. Ongoing Professional Development
C. Leadership or Teamwork
REQUIRED: Completed Checklist
